Add 'Collections Hold' stucture and hook up in Collections and modify order book process to recognize collections hold.

It would be great if coordinators had immediate visibility of customers on credit hold. As the system exists, credit holds are not noticed until revenue is generated. We should be able to request COD from customers on hold.
